Form 1A is the statutory medical fitness certificate required for transport driving licences in India. Here is who needs it, which doctor must sign it, and how to get it correctly the first time.

Form 1A is a prescribed statutory form under the Central Motor Vehicles Rules, 1989. It certifies that an applicant for a transport vehicle driving licence — taxi, auto-rickshaw, lorry, bus or commercial cab driver — is medically fit to drive on public roads.

Who needs Form 1A?

  • Applicants for a new transport (commercial) driving licence
  • Renewal of an existing transport licence
  • All applicants above 40 years of age applying for or renewing any driving licence
  • Applicants converting a private licence (LMV) to a commercial one

Who is authorised to fill Form 1A?

Form 1A must be filled by a Registered Medical Practitioner — typically a Civil Surgeon, government hospital medical officer, or RTO-empanelled doctor. Each state RTO maintains a list of authorised doctors. The form cannot be filled by a private clinic doctor unless they are specifically empanelled by the RTO.

Documents required

Aadhaar or PAN, two passport photographs, learner's licence copy (for new licences), existing driving licence (for renewals), and the prescribed Form 1A blank obtained from the RTO or downloaded from the Parivahan Sewa portal.

Step-by-step process

Download Form 1A from parivahan.gov.in, fill in your personal details, visit an RTO-authorised civil surgeon or government hospital, undergo the full examination, collect the signed and stamped Form 1A, and submit it along with your driving licence application or renewal.

Validity and renewal

Form 1A is generally valid for one year. Commercial drivers must obtain a fresh Form 1A at every licence renewal — usually every three to five years depending on age. After 50 years, more frequent medical evaluation is mandated.

Common reasons RTOs reject Form 1A

The form is rejected when filled by a doctor not on the RTO panel, signed without the official government hospital stamp, missing essential vision or hearing entries, or when the doctor's registration number is not legible. Always confirm with the RTO before paying for the examination.
