Form 3 is one of several statutory health declarations used across Indian labour, motor vehicle and educational regulations. The most common usage of "Form 3" in everyday parlance refers to the medical certificate of fitness for young persons employed in factories under the Factories Act, 1948.

What is Form 3 under the Factories Act?

Section 69 of the Factories Act, 1948 mandates that no young person (aged 14–18) may be employed in a factory unless a Certifying Surgeon has examined them and certified them fit for the proposed employment. Form 3 is the prescribed format for this certificate of fitness.

Who can issue Form 3?

Only a Certifying Surgeon — an MBBS doctor empanelled or appointed under the Factories Act by the State Chief Inspector of Factories — can issue Form 3. The list of Certifying Surgeons is maintained at the office of the Director of Industrial Safety and Health in each state.

In motor vehicle parlance, some states refer to a Form 3 medical declaration for non-transport licence applicants below 40. In educational contexts, Form 3 is sometimes used for school transfer health declarations. Always confirm the exact form being asked for — the format and issuing authority vary.

Employing a young person without Form 3 is an offence under the Factories Act, attracting fines and imprisonment for the factory occupier. State inspectors regularly audit Form 3 records as part of routine inspections.
