Medical Certificate Format for Students — What It Needs (India 2026)

Correct medical certificate format for college and school students. 7 required fields, university-wise rules, and why so many student certificates get rejected.

Indian colleges reject thousands of student medical certificates each year because they fail to include specific required fields. This guide consolidates the exact format that schools, colleges and universities expect — based on UGC, AICTE and university-specific norms.

The 7 mandatory fields for student certificates

  • Student's full name as recorded in the institution
  • University enrolment number or school admission number
  • Date of clinical examination by the doctor
  • Specific diagnosis in clinical terminology (avoid vague terms like "not feeling well")
  • Exact start date and end date of medical unfitness
  • Doctor's name, qualifications and NMC or State Council registration number
  • Doctor's handwritten signature and official clinic stamp

University-specific variations

Delhi University and JNU require certificates from CGHS-empanelled doctors or government dispensaries for attendance condonation. Mumbai University accepts private clinic certificates but requires counter-signature by a gazetted officer for absences over 15 days. Anna University and VTU often demand certificates specifically from government hospital medical officers for missed-exam re-tests.

School-level certificate format

CBSE and state boards accept certificates from any registered MBBS doctor for routine sick leave. For long-term absences (over 15 days), schools may request a counter-signature from a government doctor or civil surgeon. The certificate must clearly state the dates of unfitness.

Sample wording for student certificate

"This is to certify that [Student Name], age [X] years, [gender], student of [Class/Programme] at [Institution Name], was examined by me on [date]. The patient was suffering from [diagnosis, e.g., acute viral fever with body ache]. I have advised complete rest from [start date] to [end date]. The student should be considered medically unfit to attend academic activities during this period. Dr. [Name], MBBS, Reg. No. [State Council/NMC Number]."

Why student certificates are most commonly rejected

Backdated certificates issued after the absence; certificates without a specific diagnosis; missing dates; missing or illegible NMC number; missing clinic stamp; certificates from RMPs not registered with NMC; and submission after the institution's deadline are the top reasons for rejection. Always cross-check against the institution's template.

Best practices

Visit the doctor on the first day of illness (not afterwards). Request the certificate at the consultation itself. Photograph or photocopy the certificate before submitting. Submit within 3–7 days of returning to the institution. Retain proof of submission (email acknowledgment or stamped received copy).
