Medical Certificate Format in India: A Complete Visual Guide

India does not have a single national medical certificate format imposed by law for general sick leave — instead, the NMC and state councils prescribe the mandatory content and let practitioners and institutions decide on layout. However, decades of practice have produced a recognisable Indian format.
Header block
The header occupies the top 20–25% of the certificate. It carries the clinic or hospital logo (often a caduceus), the practice name in bold serif, the doctor's full name, qualifications (MBBS, MD, DM, FRCS), specialty, NMC or State Council registration number, and the clinic address with phone, email and consulting hours.

Footer block

Handwritten vs printed certificates
Handwritten certificates on a printed letterhead are common in small clinics and continue to be legally valid. Printed and digitally signed certificates from large hospitals and telemedicine platforms are increasingly the norm. The Bharatiya Sakshya Adhiniyam, 2023 explicitly recognises e-records as admissible evidence.
Institution-specific variations
CGHS dispensaries use a specific double-foil format. ESIC dispensaries use Forms 9, 10 and 11. Form 1A and Form 1 for driving licences are statutory forms. Universities like DU and Mumbai University have their own attendance condonation formats which the doctor must fill on the university's letterhead.
