Food handlers in India must hold an annual medical fitness certificate under FSSAI rules. Here is a complete guide to legal requirements, tests included, and how to obtain it quickly.

Under the Food Safety and Standards (Licensing and Registration of Food Businesses) Regulations, 2011, every person engaged in handling, preparing or serving food must hold a valid medical fitness certificate. Schedule 4 of the regulations makes this mandatory and inspectors check compliance during FSSAI audits.

Who needs the certificate?

  • Cooks and kitchen staff in restaurants, hotels, canteens and cloud kitchens
  • Catering personnel for events and institutional catering
  • Packaged food production line workers
  • Dairy and meat processing employees
  • Mid-day meal scheme cooks
  • Hawkers, street food vendors and tiffin operators

Tests included in the examination

  • General physical examination
  • Vision and skin inspection for visible disease
  • Vaccination status — typhoid, hepatitis A and tetanus
  • Stool test for typhoid carriers (where suspected)
  • Sputum test for tuberculosis (chest X-ray or symptomatic screening)
  • Blood tests including hepatitis B surface antigen

Who can issue the certificate?

Any Registered Medical Practitioner under the NMC or a State Medical Council may issue the certificate. Some states require it to be issued by a Civil Surgeon, government hospital, or municipal health officer for large catering operations.

How to get the certificate

Visit the empanelled doctor or government dispensary, carry an Aadhaar copy and two passport photos, complete the prescribed examination and basic blood tests, and collect the certificate the same day or within 48 hours. Total cost ranges between ₹300 and ₹1,500 depending on the city and inclusions.

Validity and renewal

The certificate is valid for one year and must be renewed annually. FSSAI inspectors specifically check renewal dates during quarterly audits and may suspend the food business licence for repeated non-compliance.

Special hygiene declaration

The certificate must explicitly state that the food handler is free from communicable diseases, skin infections, open wounds, and conditions like typhoid carrier status. This wording is mandatory for FSSAI compliance.
