A medical fitness certificate is a clinical declaration that the bearer is physically and mentally fit to undertake a specific activity or role. It is required by employers, sports bodies, transport authorities, food regulators and educational institutions across India.

What a fitness certificate must contain

A valid fitness certificate records the examinee's name, age and gender, the date and place of examination, the parameters tested (height, weight, BP, vision, hearing, cardiovascular, respiratory and where relevant blood and urine tests), and a clear declaration of fitness. It must carry the doctor's NMC registration number, signature, stamp and date.

Who is authorised to issue?

For ordinary employment, any NMC-registered MBBS doctor can issue the certificate. For commercial driving licences, only RTO-authorised government medical officers may sign Form 1A. For aviation, only DGCA-authorised Aviation Medical Examiners qualify. FSSAI-mandated food handler certificates require an RMP examination.

How long is a fitness certificate valid?

Typical validity is six months to two years depending on the purpose. Employer pre-employment fitness is usually valid for joining within 30–60 days. Form 1A for transport drivers is valid for one year. FSSAI fitness for food handlers requires annual renewal. DGCA fitness classes have specific renewal cycles.

A standard fitness examination covers general physical examination, vision (Snellen chart), colour blindness, audiometry, blood pressure, pulse, BMI, cardiovascular screening (auscultation, sometimes ECG), respiratory examination, urinalysis, and where required hepatitis B, HIV and pregnancy screening.
